Short answer: fatal overdose from LSD alone is not documented in the medical literature, but very large doses have produced serious medical emergencies — and the far more common dangers are accidents, adulterants and drug interactions, not the lysergamide itself.

What the case reports show
A frequently cited 1970s case series described eight people who accidentally insufflated an enormous quantity of LSD powder mistaking it for cocaine. All survived, but several required hospital care with vomiting, hyperthermia, respiratory difficulty and coagulation problems. That is the shape of the evidence: severe medical events, not documented lethality from the compound alone.
The risks that actually put people in hospital
- Adulterants. NBOMe compounds sold as acid have been directly linked to deaths in Canada and elsewhere. This is the leading cause of serious harm attributed to “acid”.
- Accidents and injury during heavy intoxication.
- Serotonin syndrome risk when combined with MAOIs, SSRIs or other serotonergic drugs.
- Hyperthermia and dehydration, especially in hot environments.
- Psychiatric emergencies, particularly where there is a personal or family history of psychosis.
When to call emergency services
Call 911 for a seizure, unconsciousness, chest pain, a body temperature that keeps climbing, uncontrolled vomiting, or any situation where someone may harm themselves. Canada’s Good Samaritan Drug Overdose Act gives legal protection from simple possession charges to people who call for help at an overdose.
