Short answer: they are not the same class of drug. LSD is a classic serotonergic psychedelic acting mainly as a 5-HT2A agonist; MDMA (“molly”) is an entactogen that forces the release of serotonin, dopamine and noradrenaline. Their risk profiles differ substantially, and MDMA’s is the more physically demanding of the two.

Mechanism
LSD binds and activates serotonin receptors directly. MDMA works largely by reversing the serotonin transporter, dumping stored serotonin into the synapse — which is why depletion, and the low mood that can follow in the days after, is a recognised MDMA phenomenon and not an LSD one.

Physical risk
MDMA carries documented risks that LSD does not: hyperthermia, hyponatraemia from overdrinking water, cardiovascular strain, and serotonin syndrome when combined with SSRIs, MAOIs or other serotonergic drugs. Overheating in hot, crowded venues is the mechanism behind most MDMA-related emergencies.
Adulteration patterns
Both are frequently misrepresented. Pressed pills and powders sold as molly have been found to contain cathinones, methamphetamine and, increasingly dangerously, fentanyl analogues. Blotter sold as acid has been found to contain NBOMe. Drug-checking services in several Canadian provinces exist precisely because of this.
Legal status
LSD is Schedule III and MDMA is Schedule I under the Controlled Drugs and Substances Act. Both are illegal to possess, produce or distribute in Canada outside authorised research.
